ASQ-3: Ages & Stages Questionnaire

ASQ-3 is a quick, parent-completed check of your child’s development from birth to 66 months. It looks at communication, movement, problem-solving, and everyday social skills—so you can celebrate strengths and catch concerns early.

Why use ASQ-3?
  • Spot concerns early. Early identification leads to timely support.
  • Whole-child view. It covers communication, gross/fine motor skills, problem-solving, and personal-social skills.
  • Parent-led. You know your child best; the tool is simple and mobile-friendly.
  • Track progress. Repeat at key ages to watch growth over time.

What happens after you submit:

A navigator reviews your results, explains what they mean, and, if needed, helps with the next steps (activities, re-screening, or referrals you choose).

Recommended Screening Schedule- The ASQ-3 offers multiple screenings tailored to different ages, ensuring continuous monitoring of your child's development. Screenings are recommended at the following ages:

  • 10 months
  • 12 months
  • 14 months
  • 16 months
  • 18 months
  • 20 months
  • 22 months
  • 24 months
  • 27 months
  • 30 months
  • 33 months

  • 54 months
  • 60 months

The Coös Coalition and Ammonoosuc Community Health Services are dedicated to ensuring that all our young residents in the North Country are screened as early and as often as possible. We offer these screenings free of charge through various organizations.

If you have a child aged 0-5 who you would like to screen, please contact us here. We will ensure that happens. Additionally, we can provide age-appropriate resources and tips at no cost to support your child's development.
